Memberships can be paid upfront or monthly based on a set schedule. Each plan includes up to 2  visits per month (in-person or telehealth), with the flexibility to choose what works best for your needs.

All memberships include:

• Wellness check-ins (in-person or telehealth)

• Comprehensive lab panels (CBC, CMP, HbA1c, Lipid Panel; based on package length)

• Medication review & management

• Nutrition and weight guidance

• Body composition analysis & Bio-Wellness Scans

Our telehealth memberships give you direct access to your provider through secure virtual visits. Whether you’re managing a chronic condition, need a quick follow-up, or simply prefer the comfort of home, telehealth keeps you connected.

Membership includes:

• Up to 2 telehealth visits per month

• Prescription management (when appropriate; some conditions may require labs or in-person visits)

• Lifestyle medicine guidance (nutrition, exercise, weight support)

• Faith-centered, personalized care

Prices​

• 3-Month Membership — $75 per month (total $225)

• 6-Month Membership — $65 per month (total $390)

• 12-Month Membership — $60 per month (total $720)

Best for: Busy professionals, caregivers, frequent travelers, or patients who prefer virtual access for routine care and follow-ups.

Available only with a 12‑month Chronic Care Package

• Adult Membership (required):

  • 1 adult: $75 per month (chronic care package required)

• Children’s Add-On (ages 2–17):

  • Add up to 3 children for $30 per child per month

• Adult membership is required for all children:

  • A child cannot be seen in our practice without an active adult membership in the same household.

• Vaccine policy:

  • Our office does not offer vaccines at this time. We are happy to discuss your options and help coordinate vaccines elsewhere if you choose.

Our a la carte services are available to everyone and can be scheduled as needed.

• Telehealth Visit (Non-Member) — $100
  Virtual consultation for minor illnesses, follow-ups, or medication questions.

• Initial Consultation — $150
  Comprehensive review of your health history and current needs.

• Chart Review (In-Person or Telehealth) — $50
  Brief review of outside medical records or recent labs.

• BioWellness Scan (Biomarker Testing) — $50
  Non-invasive scan to assess nutritional and antioxidant status.
